For New Students

9 a.m. – 3 p.m.

Please stop by or call the front office to make an appointment to register your student for the  2009-2010 school year & pick up a registration packet. Registration will be held Monday thru Thursday by appointment only. Registration will also take place during our Open House on Thursday, August 6th.

Listed below is information necessary for registration:

  1. Only the parent(s) or legal guardian(s) of the student can register their child. Legal guardians must bring guardianship papers.
  2. Proof of residency which includes one of these documents: Homeowners Contract or Lease/Rental Agreement and one utility bill (gas, water or electric). Telephone, cable, waste removal bill or a driver’s license are not acceptable documents.
  3. If you currently do not own, rent, or lease your place of residence, a notarized Residency Affidavit is required. This form is available at JMS
  4. Current Georgia Immunization Certificate (Form 3231)

- 6th grade students must have 2 MMR & 2 Varicella vaccines or proof of the chickenpox disease before they can enroll.

  1. Georgia Hearing, Vision, Dental Certificate
  2. 08-09 final Report Card showing grade promotion
  3. Copy of Discipline record
  4. Student’s Birth Certificate
  5. Copy of Standardized Test scores, if possible
  6. Gifted eligibility
  7. If Student receives special education services, a copy of their most current IEP
  8. ESOL Students (Speakers of Other Languages)-International Center Documentation and ESOL Documentation. If you don’t have this documentation, please call the INC at 678-985-5200 to set up an appointment for your student to be tested.
